documentation for import pets to Cambodia

Hi everyone, please help if you can, I am relocating from Thailand to Seim Reap Cambodia with my dog, I need to know what paperwork the customs will require for my pet to gain entry. I am getting very mixed results here in Thailand and no answers from Cambodia gov. website. Going overland through poipet boarder. Leaving A.S.A.P, so any info would be much appreciated.
