    Participant users are required to hold a valid Lebanon Dog Permit. Dog Permits are issued at the Lebanon Parks Department office at no charge. Members must purchase a Dog Park membership each calendar year at the Lebanon Parks and Recreation Department, 130 E Ulen Dr, Lebanon, Indiana 46052.

    Proof of current vaccinations is required. Rabies, Leptospirosis, DHPP (Distemper, Hepatitis, Parainfluenza, Parvovirus, and Bordetella). Proof of current vaccinations must be completed by a veterinarian or animal clinic.
