Travelling with my dog to the united states

This will be my first time on a plane and I am bringing my dog with me to Boston.
She is a 65lb pitbull cross and I have a very large crate for her.
I am planning on driving to Seattle and taking a plane from there, I think it would be cheaper. Anyone have a rough idea on how much it will cost for her?
I know she needs to be completely up to date on her shots as well as have her rabies...is there anything else she needs? And what kind of proof of her shots do I need? Any information at all would be helpful, this is all totally new to me!
