Documentation that doesn't cost an arm and a leg

We are traveling to Mexico again in January 2017. Last year our vet (Pet Smart) charged $187 to fill out the forms. Then we had to send those forms to our State capital to get them approved, which cost us more money. Does anyone know a cheaper way to get this work done?
