Traveling with esa

I recently registered my dog as my emotional support animal and I plan on flying with her soon. However she is 45 pounds and doesn't fit under my seat. What are the policies for flying with a dog that large?

The airline I am flying with, Westjet, has a policy that dogs can be designated as lap held by Doctor as long as they are no larger than a 2 year old child. Not sure if that helps. 45 pounds on your lap for several hours could be tough.
